ADVANCING HEALTHCARE PRACTICE Small grants awarded during 2004 Developing the Skills of Nurses to Improve the Quality of Patient Discharge from Hospital
Project leader: Liz Lees Location: Birmingham Heartlands Hospital Summary of initiative: A national tool-kit to facilitate nurse-led discharge is currently at the penultimate stage of development (Lees, 2004). This project envisages getting evidence from this into everyday healthcare practice by activating the core competencies within the tool-kit, to the benefit of staff and patients. This will involve working in a clinical capacity with sixteen qualified nurses (D to G grade) and the multi-disciplinary team, in an elderly care ward at the Birmingham and Heartlands Hospital. The aim is to support staff by facilitating training/reflection/competency development to improve their practice of discharging patients from the hospital, through nurse-led discharge activities.
