You can complete the form by:

•  Printing the form off your computer and completing the form fields by hand

•  Saving the form to your computer and completing the form fields electronically

The annual subscription fee is £40 and there are currently two payment options.

Payment by cheque:

• Please complete the Subscription Form, indicating that you are paying by cheque (see Box 9 of form). Cheques should be made payable to The Foundation of Nursing Studies.

• Please return the Subscription Form and cheque to the address provided below.

• Once we have received payment, we will enter your details onto the database and then contact you by email to give you your password.

Payment by invoice:

• Please complete the Subscription Form, indicating that you are requesting an invoice (see Box 10 of form). Please provide clear details of who should be invoiced e.g. name of organisation, department etc. and where the invoice should be sent if this is different from your own address.

• Please return the Subscription Form to either the postal address provided below or if you have completed the form electronically, it can be sent as an email attachment to the email address below.

• Once we have received payment, we will enter your details onto the database and then contact you by email to give you your password.

Team subscriptions:

We are able to offer a discount for team subscriptions - 5 subscriptions for the price of 4. Please complete 5 subscription forms and include payment for 4 subscriptions.
